General
Summary:
The Direct Support Professional (DSP)/Contracted Support Staff (CSS) will provide face to face in-home and community services for individuals who live with intellectual and developmental disabilities.
The vision is to facilitate services according to each Consumers Individual Support Plan (ISP) with the main focus being skill development or maintenance in areas including functional, social, and daily living. The services provide guidance to promote independence, interdependence and community integration. Services occur at scheduled times, and can be implemented in the individual's home or community settings.

Core Essential Duties and
Responsibilities:
Ensures Consumers' health, safety, dignity and individual rights while enhancing the quality of the individuals' everyday life.
Participates in the development and implementation of each Individual Support Plan (ISP).
Provides service to the Consumer at home and/or their community according to their Individual Support Plan (ISP).
Participates in implementing the Individual Support Plan (ISP) designed to enhance the individual's social, emotional, cognitive, physical, and independent living skills.
Assists individuals in the development of appropriate behavioral, social, money management, housekeeping, hygiene, and other skills based on outcomes in the Individual Support Plan (ISP).
Adheres to Department of Public Welfare (DPW)/Office of Developmental Programs (ODP) Regulatory requirements Tittle 55 Chapter 51 Office of Developmental Programs Home and Community-based Services.
Completes and submits all daily progress notes, and time sheets in a timely, and professional manner in accordance with CFP
guidelines.
Collaborates and cooperates with involved county agencies, family members, natural supports network and other service providers; participates in meetings and provides relevant information.
Transports Consumer's in direct support staffs personal vehicle for community outings or scheduled appointments.
Works independently with minimal supervision in the Consumer's home or local community.
Ensures that the services are provided incorporating the Principles of Everyday Lives and Positive Approaches in the least restrictive setting and with the utmost attention to the health, safety, rights and dignity of the Consumer.
Provides support in medical related emergency situations according the CFP
Consumer Medical Emergencies Policy.
Reviews and assesses initially and on an ongoing basis the Individual Support Plan (ISP) in order to provide appropriate services to the Consumer.
Provides support and assistance to individuals in arranging for medical care when responsible to do so as per the Individual Support Plan (ISP) and follows health care recommendations.
Facilitates skill instruction for the Consumer in an appropriate setting according to each Consumer's Individual Support Plan (ISP).
Utilizes community resources to promote community integration, independence, and interdependence.

Ability to understand and relate to persons with disabilities.
Ability to learn and apply positive approaches when supporting individuals.
Ability to maintain information in a confidential manner
Ability to apply the principles of Everyday Lives and Positive Practices person first language.
Ability to observe and assess the needs of individuals.
Ability to maintain professional and respectful interaction with the Consumer, their family, supports network and other CFP
staff.
Ability to organize, prioritize and multi-task work assignments.
Ability to work alone with minimal supervision.
Knowledge and the ability to utilize good listening skills and comprehend verbal instructions given in English.
Ability to read and comprehend instructions written in English.
Ability to write legibly in English in a clear, concise, and logical manner.
Ability to speak clearly and express self effectively in English.

Minimum Education, Credentials, and
Experience:
High school diploma or equivalency or have the knowledge, skills and abilities to perform the functions of the position.
Ability to perform first aid/CPR is preferred but not a requirement

Special Requirements:
Must be able to work flexible hours according to the Consumer's Individual Support Plan (ISP).

Must have a valid a current driver's license, vehicle registration, vehicle insurance and access to a reliable vehicle to attend trainings, meetings, and/or transport Consumer to appointments.

Must be able to travel to trainings/meetings upon request of program director.

Direct Support Professional (DSP)/Contracted Support Staff (CSS) is an independent contractor! What that means for you is:
You are responsible for your own Federal, State and Local taxes, you will receive a IRS Form 1099-Misc from CFP
by January 31 of the next year. Consult a tax professional for further guidance;
You are not covered under our Workman's Compensation;
You are not eligible to apply for Unemployment Compensation under CFP+;
Your time required to accomplish annual Provider Qualification trainings is not paid for by CFP+.
Must be able to accompany individuals into the community and provide support and assistance in interactions in the community while maintaining sensitivity to the rights and dignity of the individual.
Must have the ability to obtain clearances as defined by DPW/ODP regulations.
